Nursing Job Shadow Day Program

11/3/2009 NOTICE: University Hospital is taking precautionary steps to prevent the spread of the H1N1 flu. Therefore, until further notice, all job shadowing programs and all internship programs have been suspended at University Hospital.

This restriction does not apply to students cleared through Employee Health or who are covered under formal contracts that University Hospital has with accredited Colleges and Universities.

On-Line Job Shadow Day Program Registration is Temporarily on Hold Until Further Notice.

SUNY Upstate Medical University is pleased to offer students a chance to explore nursing in a dynamic clinical setting.

Each Shadow Day participant will be paired one-on-one with a health care professional in one of the following nursing fields:

• Ambulatory Surgery & Procedures - Outpatient Procedures
• Cardiovascular Services - Heart and Blood Vessels Conditions
• Emergency Services
• Medicine Services - Kidney and Lung Related Conditions
• Neuroscience Services - Brain and Spinal Cord Conditions
• Orthopedics - Bone Related Conditions
• Pediatrics Services - Infant & Child Related Conditions
• Peri-Operative Services - before, during and after surgery.
• Inpatient Rehabilitation

Participants will interact with the professionals as they perform their normal daily activities. After the job shadowing, participants will attend an afternoon reception to share their experiences and meet Upstate faculty and students.

To Be Completed Upon Acceptance

• Assumption of Risk Release Form
• Upstate Medical University Confidentiality Agreement
• Upstate Medical University Detailed Health Statement (required for a visit of more than 3 days)
• Upstate Medical University Short Term Health Statement (required for a visit of less than 3 days)
• Signed HIPAA Privacy Rule Education Completion Certificate (Must first complete HIPAA Self Study presentation).
