  • Complex discharge planning
    • Facilitate inpatient psyciatric hospitalization

  • Crisis: intervention (assessment/stabilization/problem solving)
  • Grief and Loss Resources
  • Advance Care Planning: Assist with conversations/decision making assistance regarding Health Care Proxies, MOLST, Living Wills, DNR, and Assuring Your Wishes
  • Short-term Counseling Intervention
  • Substance abuse evaluation and referrals
  • Violence: Suspected family violence, including domestic violence, child abuse, and elder abuse
