Referrals

Patients are accepted at the center by written referral. In order to expedite a complete patient evaluation, referral letters should include the following:

• patient's pain problem
• medical history including diagnostic tests
• present medications
• insurance data
• address
• telephone number.

Upon receipt of this information, the patient will be contacted and an appointment made, usually within two weeks.
