Making an Appointment

All patients need to be referred by their physician.

New Patients

All new patients are required to bring all:

  • Available records if not being send by their physicians office
  • X-rays
  • List of medications
  • Insurance card to their initial visit

New patient consults should be prepared to spend a minimum of 1 hour in the office for an office visit which includes preparation, physician time, and discharge time. Subsequent visits will usually take less time, dependent on whether the patient is scheduled for a diagnostic test on the same day as their appointment.

Cancellations and appointments which must be rescheduled can most efficiently be handled by calling the necessary office.
