High School Seniors and first year college students
applying for early admission are not required to complete

GRADUATE APPLICANTS

DPT Program — Applicants must complete the Doctor of Physical Therapy Application

Medical Technology Program — Applicants must complete the College of Health Professions Application ONLY.

Apply on-line to AMCAS (American Medical College Application Service — http://www.aamc.org/audienceamcas.htm)

Request that their application be transmitted to Upstate Medical University. AMCAS is a non-profit, centralized application service for students applying to their first-year of medical school.

Transfer Applicants to the College of Medicine must complete the College of Medicine Transfer Application

Early Assurance Applicants to the College of Medicine must complete the College of Medicine Early Assurance Application
