Referring Physicians

Scheduling Tips

Please be prepared to give our scheduling team the following information when scheduling an exam at one of our URAIC sites:

  • Name
  • Social Security number
  • Date of birth
  • Exam and diagnosis
  • Name of referring physician
  • Insurance carrier name
  • Insurance carrier subscriber name
  • Contract number
  • Group number

When scheduling an appointment, our staff will advise you to remind your patients to bring any necessary paperwork with them including their Insurance I.D. card. If this is a patient's first visit to one of our sites, it is important for them to arrive at least 15 minutes prior to their appointment so the necessary paperwork can be processed.

We would ask you to give us a 24 hour notice if an appointment must be canceled and we will be happy to accommodate your patient's needs.

We welcome patients to give us a call if they have specific questions regarding the exam they will be receiving. We would also ask that all patients bring any available comparative films/CDs.

PLEASE NOTE— We require an authorization for all MRIs for most insurance carriers before the exam can be rendered. It is important for scheduling staff to verify your patient's insurance coverage and obtain the authorization before the appointment is scheduled with our staff. We are more than happy to assist in this process .
