Appointment Information

General Information

Apppointments are scheduled Monday through Friday from 8:00 a.m. to 4:30 p.m. To schedule an appointment with one of our physicians, please call our main line 315-464-4472. If you do not have a specific physician referral, we will do our best to obtain the earliest available appointment with the physician most familiar with your type of problem/injury. For your convenience, we have provided the direct phone numbers to each of the physicians within our group on the medical staff page.

If you are unable to keep your appointment, please give us at least a 24 hour notice.

We make every effort to honor all time commitments. Unfortunately, because of unanticipated problems and emergencies, your appointment may sometimes be delayed. We will make every effort to keep such delays to a minimum and will keep you informed of them as they occur.

Our goal is to provide you with the best possible medical care. Because this is a teaching facility, residents and medical students may be involved in your care under the direct supervision of your doctor. You may also be seen by a Nurse Practitioner when the physician is unavailable.

New Patients

Prior to your scheduled appointment, please be sure to complete our pink registration form mailed to you. If time permits, please mail this form back to our office. If you have any pertinent records, such as x-rays, procedures, surgeries, etc., call your corresponding physician's office to request that a copy of these records be sent to our office. This additional information is very important to you and our physicians and will assist them in determining the treatment plan most appropriate for you.

Appointment Checklist

Prior to your appointment you may want to make sure of the following:

1. Referrals or authorizations have been obtained.
2. Your pink registration form is completed and returned to our office.
3. Necessary insurance forms and insurance identification cards are brought with you.
4. Medical records and/or Radiology films from other physician offices have been requested and sent to our office.
5. You have a pre-written list of questions you may want to ask your doctor.
6. If it becomes necessary to cancel this appointment, please call the office to let us know as soon as possible.
