Appointments for new patients or for patients, who had transferred care to another provider and wish now to return for services offered by the DAC, can be made by the patient or by a physician referral. To make a new patient or returning patient appointment, please call 315- 464-5533 and follow the prompts.
Established patients may contact the clinic at 315 464-5533 and follow the prompts to schedule appointments.
For our non-English speaking consumers interpretation services will be provided.
We work with many local and regional HIV programs and other human service providers. We will help you with referrals as needed.
First time clinic visits may take a few hours. Please plan with this in mind if you are in a pay parking lot or if someone is giving you a ride. Your first visit will include talking about your medical history, a physical exam and lab work. You will also meet with one of our social workers and perhaps our dietician and other service providers as needed.
If you are unable to keep a visit time please call to cancel and/or reschedule. Your visit time can then be used by someone else. If you are going to be late for your visit please call us at 315-464-5533 and press 0 to speak with the secretary. If you are 20 minutes or more late for your visit you may be asked to reschedule.
We cannot provide childcare during clinic visits or group sessions.
We request that cell phones be turned off once a patient has been placed in the exam room.

Rosemary Rochford, PhD has been named Vice President for Research. Dr. Rochford, a member of the Upstate faculty, previously served as Chair of the Department of Microbiology and Immunology.
