New Employee Checklist
Bring to Your Pre-Employment Visit:
Your physical will be scheduled before you begin work. Please bring the completed forms below to your physical:
- Medical History Form
- Allergy Screening Form
- Medical Staff Office Health Clearance
Nurse practitioners, Physician Assistants, Nurse Anesthetist, Nurse Midwives only, instead of the above forms, use this form: Medical Staff History and Physical Form
Required Forms/Credentials:
- Diploma, license, if applicable
- Please note: Non-licensed applicants who have completed their education outside of the U.S. must have their foreign education evaluation by an accredited organization to ensure that the foreign education is comparable to education received in accredited educational institutions in the U.S. To be acceptable, the foreign credential evaluation must be from a member organization of one of the two national associations of credential evaluation services:
- National Association of Credential Evaluation Services (NACES) - https://www.naces.org/
- Association of International Credentials Evaluators (AICE) - http://aice-eval.org/

Payroll Documents
- Voided personal check if you want to sign up for direct deposit (recommended)
Parking Documents
- If you are signing up for parking, bring your vehicle registration and first month's payment. Monthly rates
Benefits Documents (Only for New York State Employees)
Required for each benefits enrollee/dependent:
Employee:
- Birth Certificate* AND/OR Valid Passport AND Social Security Card
- Birth Certificate* AND/OR Valid Passport AND Social Security Card
- Marriage certificate/license (in addition for marriages dated more than one year prior, proof of current joint ownership/financial obligation. If adding a Domestic Partner please ask for the domestic partner packet.
- Birth Certificate* AND Social Security Card
*You must provide a Birth Certificate from the Bureau of Vital Statistics. To replace your original birth certificate for yourself or you eligible dependents go to (based on State or Territory of birth within the US): http://www.cdc.gov/nchs/w2w.htm

You can complete the above forms before your New Employee Orientation. If not, you will be asked to complete them in person at the Human Resources Office, or during orientation. If you have problems accessing these forms, please call your recruiter. You can fill the forms out online and then print them or print them out and fill them in by hand.

Notice Regarding Health Plan Marketplace
- Notice for New York State Employees ONLY: This is a required notice that provides information to all NYS employees about the availability of the health plan marketplace. Although it's unlikely those NYS employees eligible for health benefits will find a better deal on the marketplace, it's important information for those not eligible for NYS health coverage, or for those who lose NYS coverage due to termination or those dependents who "age out" of coverage under their parent's plan. Those who wish to learn more about purchasing coverage through the marketplace are directed to the federal government Web site: www.healthcare.gov.
