Making an Appointment

Appointments are primarily made by physician request for consultation. Self referrals are accepted with proper supporting clinical records or for patients who are interested in a second opinion.

New Patients

Appointments will not be scheduled until we have received the following information:

  • Applicable records, including labs
  • X-rays
  • Patient demographic information, including a copy of the insurance card Patients should bring with them to the first appointment:
  • A list of all medications, including over the counter medications
  • A form of picture ID, eg. a drivers license
  • Their current health insurance card(s) and any drug insurance card
  • The completed history form that was sent in the mail

New patient consults should be prepared to spend a minimum of 1.5 hours in the office for an office visit which includes preparation, physician time, and discharge time. Subsequent visit times will depend on the patients diagnosis and plan of care and this will be discussed with you at the initial visit.

Emergencies

A physician is available by phone 24 hours a day and is reached by calling the Regional Oncology Center main phone number at 315-464-8200. When patients call the appropriate phone number, their call will be transferred to the answering service who will relay the message to the on-call physician. The on-call physician or resident will respond to the call as soon as possible for further guidance.

Life-threatening Emergencies

Patients should call 911 for an ambulance to transport them to the nearest hospital or emergency department.
