Alumni Questions:

I served as a resident at the SUNY Upstate Medical University, but I have lost my Residency Certificate.
How do I get a new copy?

Send your request in writing to:

Syracuse, New York 13210

Enclose a check for $60.00 (for each certificate requested) made out to Upstate Medical University. We will mail you a new Residency Certificate within 2 months.

How do I go about verifying a residency done at Upstate Medical University?

Enclose a signed release form along with your request. Please allow 4 weeks for processing.
