Case Management

Pediatric case managers are specially trained pediatric nurses who assist children and their families in making their transition from the Hospital to Home as seamless as possible. In addition, your case manager will also assist you with accessing resources and services within the hospital while your child is hospitalized.

Clinical Services Provided

  • Transition children to inpatient rehabilitation (acute/subacute) facilities when needed

Support Services Provided

  • Act as a pharmacy and infant formula liaison and ensure that any difficult to find and/or costly medications and/or infant formulas are available prior to discharge. Coordinate with the medical team to complete prior authorizations if required by th
  • Assess and verify health insurance coverage. Advocate on the patientís behalf as needed and assist uninsured families.
  • Coordinate appropriate resource utilization and communication between team members while the child is hospitalized.
  • Ensure that child and family have safe transportation to home and appropriate car seat when indicated.
  • Make appropriate home care, home infusion and durable medical equipment referrals for safe transition home.
  • Meet and assess 100% of admissions within 24-72 hours. Explain role of case manager and assess needs for discharge.
  • Participate in multi-disciplinary rounds and make referrals to services as needed.
  • Refer to Early Intervention, schools, outpatient therapies, WIC, developmental service organizations or other agencies as needed for continuation of care in the community setting.
