Health Clearance

An important reminder is that you have your student health file completed in full by August 1, 2007. In accordance with New York State public health law, you will not be allowed to officially register for courses, August 21st during campus orientation or attend classes beginning on August 25th unless complete.

If you are a present or past employee, student or volunteer at Upstate Medical University, please contact the Employee Student Health Office before completing any of the health information. Some of the required health information may already be on file.
