Case Management

The case management department is staffed by registered professional nurses and social workers. Each patient at Upstate University Hospital Community Campus has a dedicated team comprised of one nurse and one social worker.

Together, the case management team works to:

  • Facilitate patients' safe and timely discharges.
  • Promote quality and cost effective outcomes
  • Collaborate with the patient, family doctors and nurses to facilitate timely, safe discharges.

The team of social workers is available to assist patients with:

  • Adoptions
  • Advance directives
  • Chemical dependency
  • Child abuse/neglect
  • Domestic violence
  • Elder abuse
  • Hospice/end of life care
  • Mental health issues

We encourage patients and their families to participate and cooperate with the case management staff as they facilitate a safe patient discharge.
