This physical therapist is responsible for providing in-home physical therapy to patients with a referral for physical therapy and by request of the patient or physician for homevisits. The physical therapist will conduct initial evaluations, assessments, establish a plan of care and implement high quality, evidence-based interventions to promote functional improvements, maximize patient independence, and improve the patient’s ability to attend outpatient physical therapy.
Possess at least a Master’s degree in Physical Therapy and a current and valid NYS License in Physical Therapy. One year of clinical experience is preferred but new grads are welcome to apply. Experience in providing home physical therapy and/or outpatient services are a plus. Current NYS driver license, a reliable vehicle to travel to home visit and/or to the physical therapy clinic as necessary and current proof of updated vehicle insurance.

Job Summary: The Physical Therapist provides services to patients/clients, in an inpatient acute rehab setting, who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury (ie, MMT, fall, amputee, etc), disease (cardiac, pulmonary, gastrointestinal, integumentary, etc), or other causes. Primary patient population is of a neuro diagnosis, including but not limited to SCI, CVA, TBI, Parkinson's, and MS. Restores function, provides prevention education, promotes health, wellness and fitness, alleviates pain and prevents/ minimizes disability.
