• Portfolio and Review Fee
  (Required of anyone desiring to apply for transfer credit)
• College Transcripts (Upstate Transcripts are NOT required)
• One Letter of Recommendation
• Copy of US Physical Therapy License
• $50 Application fee
• $250 portfolio review fee (if portfolio is required)
